The war Graves Photographic Project have photographed his headstone.You can obtain a photo if this is something you would like, however they do ask for a donation, £3-50 for an emailed version or £5-50 for a hard copy sent in the post

From his entry on Distinguished Conduct Medal Citation:-

For conspicuous courage and devotion to duty while employed as scout and orderly,especially at St. Eloi when, after being wounded in the thigh, he completed the journey he had undertaken and delivered his message (30.6.15)

EDIT - SORRY, I think this must be a different S.H.Middleton as this one is a sargeant and yours is a sapper.

Hello, I really would appreciate some help on searching for military records of Samuel Hurworth Middleton. He was born Moorsholme Yorkshire in 1890 to Mary Jane and William, I also have him on 1891,1901 and 1911 census, I also have his marriage details. I am interested in his military career, and have found his details on CWWG site where I found he died during !st WW, I also have his medal index card. I cannot find his service records or his pension records and thought may be a fresh pair of eyes could help...it may be I know, that his records were one of those that were destroyed. Any help would be gratefully accepted... Thank you :-\
